Consumer Direct Care Network provides services and supports that help people and families of all ages including children, older adults and those with disabilities, remain safe, healthy and independent in their own homes. We value the strength and ability of each person. We believe that by helping manage your own care, we contribute to your health and help build healthier, happier communities. We focus on the possible by providing you a coordinated care group that offers in-home services. We take the time to find out your needs, what programs you qualify for, and how we can assist you in meeting your in-home care needs. If you are not certain what type of care is necessary, our seasoned team of professionals can expertly assist you in choosing appropriate supports and/or services.
