As you walk the path of life, needs change. Some tasks become increasingly difficult and extra help is needed. Consumer Direct Care Network New Mexico provides assistance to stay in your home when a disability or functional challenge becomes too much to handle alone.
If you are uncertain which type of care you need, our veteran team of professionals can expertly assist you in choosing the appropriate level of care. This means you have more choice and control over the care you receive and who provides that care.
Personal Care Services-Directed Model
This directed service option gives you more choice and control over the care you receive and the person who provides that care. You recruit, select and train your caregiver and we are here to assist you. You manage the caregiver’s daily work and sign-off on his or her timesheet.
Caregiver time is logged through Electronic Visit Verification (EVV). EVV eliminates paper timesheets by providing real-time, point-of-service capture of caregiver shifts through a smartphone or telephone landline, validation of participant eligibility at time of service, and integrated billing.
If you are eligible to receive Personal Care Services, we can assist with these activities of daily living:
- Individualized bowel and bladder services, including toileting
- Meal preparation and assistance
- Eating
- Household support services, including shopping and errands
- Hygiene and grooming, including help with dressing, physician ordered skin care
- Supportive mobility assistance, ambulation, transferring, and repositioning
Personal Care Services-Delegated Model
When you choose this option, we take care of all of the details for you. We have a trained and experienced group of Personal Care Attendants (PCAs) that are ready to assist you. We also provide back-up care should your assigned PCA call out for a scheduled shift.
